I have a FreeBSD mail server down my basement, it runs PostFix, SpamAssassin, RAV and ClamAV. 99% (or more) of spam is being filtered. One false negative per week or less.

SpamAssassin gives you the same functionality, and it's free. Plus, there are Mac OS X-specific instructions for installing it. The bas news is that it's a bitch to install. I don't run it on my Mac; instead, I use a Linux box in my basement as my mail server, and I have SpamAssassin installed on there. I filter all my spam to a separate "spam" mailbox, and usually end up with 99% of spam going there. I have roughly one piece of spam every two days make it through to my main mailbox, while hundreds of spams are flushed down the tubes.
